Job Description
Duties & Responsibilities
NURSING CARE
- Utilize nursing knowledge and skills implementing basic preventative therapeutic and rehabilitative nursing care of assigned clients
- Perform delegated independent nursing functions using established procedures, policies, guidelines, and standards
- Administer medication accurately, observing client response, as evidenced by documentation in the medical record
- Accurate charting of medication administration
- Proper disposal of medication
- Perform limited I.V. therapy procedures on those clients receiving peripherally inserted intravenous fluids, as delegated by and observed by an RN (Community Hospice Care)
- Verify the type of solution being administered
- Evaluate the infusion site and the extremity for possible infiltration
- Verify the intravenous according to the prescribed flow rate
- Discontinue the intravenous at the appropriate time
- Taking vital signs such as blood pressure, weight and temperature
- Basic wound care including cleaning and bandaging injured areas
- Bowel care
- Monitoring fluid and food intake and output
CLIENT ACTIVITIES OF DAILY LIVING
- Assist client with showering, bathing and bed washes
- Assist client with oral hygiene, brushing teeth, and cleaning of dentures
- Assist with dressing and grooming of clients
- Observation of basic skin integrity & reporting skin condition to the Manager or Nurse Team Lead
- Facial shaves of male clients, and hair care
CLIENT MOBILITY
- Assist clients to transfer from bed to chair
- Assists clients to change position in bed
- Range of motion (ROM)
- Assists families with client transfers and manual handling (including mechanical lift)
CLIENT TOILETING
- Assist clients to toilet
- Provide bedpan/urinal
- Emptying, measuring and recording urine from urine collecting drainage bags
OTHER
- Maintain the confidentiality, privacy and dignity of clients
- Demonstrate empathy towards clients and their families
- Act tactfully and diplomatically with sensitive situations
- Risks exist in all settings, as such ensure tasks you complete are done so in a safe manner, according to New Dawn Homecare policies to always protect the safety of clients
- Timely reporting of all safety incidents by notifying Manager immediately
- Follow proper protocol on hand washing and hand hygiene techniques
- Follow infection prevention and control procedures such as wearing PPEs when necessary
- Be conscientious of any hazards in the home environment and to act accordingly in meeting the safety and protection needs of the client
- Active involvement in New Dawn Homecare Violence Prevention and Control program to ensure you recognize workplace violence, minimize and prevent it, and respond appropriately if it occurs
- Encourage clients and families to be involved in care, if appropriate
- Observe, document, and report any emotional or physical changes noted in the client’s condition to the Manager or Nurse Team Lead
- Complete charting documents for every shift and submit to the office on a weekly basis
- Organize time and resources based on the individual needs of clients
- Maintain the confidentiality of clients and organizational proprietary information
- Participate in internal team meetings as required
- Look for potential improvements and help implement solutions
- Maintain quality, safety and the confidentiality of employees, clients, and volunteers
- Maintain professionalism, tact, diplomacy, and sensitivity so as to portray New Dawn in a positive manner, consistent with its values and vision
- Update job knowledge by participating in educational opportunities, engaging in professional networks, and participating in professional organizations
- Respond to the needs of others with effective communication, mutual respect, and consistent follow through to build trust and workplace relationships
- Promote the mission and values of New Dawn both internally and externally
- Other duties as assigned by the Manager New Dawn Homecare
Qualifications
- Registration with the Nova Scotia College of Nursing (NSCN) and graduate from an accredited school of nursing
- Minimum one (1) year nursing experience in a community setting
- Valid and up-to-date First Aid & CPR certification
- Clear criminal records check

Work Conditions
- Some travel may be necessary
- Some bending and lifting is required to clean/maintain home
- Manual dexterity is required to use desktop computers and peripherals
- Some evening and weekend work may be required
- Overtime as required
